- terms and conditions
These
Terms and conditions constitute an agreement between you (Patient)
and Medica Travel & Consulting., registered in Poland referred
in this document as Medica Travel. By booking any of our treatments
and by arranging a consultation, treatment or surgery with one of
our Clinics or Hospitals of which Medica Travel represents you.
You confirm that you have read and understood these terms and conditions
and have agreed to come into the agreement to this conditions.
Medica Travel
Medica Travel is a Poland-Based health agency
that organizes cost-saving medical trips to Poland. We have long
experience of dealing with European Patients seeking affordable
and high quality medical treatments in Poland. Within our full service,
we provide all forms of medical treatment, from dentistry to invasive
surgery.
Medica
Travel website is owned and operated by Medica Travel & Consulting,
registered in Poland, under REGON 220-21-60-50, NIP 584-25-85-162

We
collect and translate Patient's medical records and other medical
information and forward them to Clinic or Hospital. |

We
forward any questions or inquiries a Patient might have to dentist
or doctor. |

We
book hotels, tests and surgery for our Patients. |

English speaking staff from Media Travel are available
around the clock seven days a week. |

We
assist Patients during their stay in Gdansk. |

We
transfer Patient between airport, port, hotel and Clinic/Hospital
(in Gdansk). |

We
follow-up the Patients' treatment/surgery after returning home.

Price of our
Service
 |
Our
service is included in the price of treatment. Services within section "health care" are charged according to current pricelist. |

All
medical costs involved in the treatment are specified in the
offer and in the confirmation. No hidden costs. |

Patients
who book their treatment in Gdansk will receive transfer free
of charge between airport, hotel and Clinic or Hospital for
all appointments (patient can bring maximum one companion free
of charge). |
Inquiry, Booking
and Confirmation
Your doctor or dentist will need to assess
your state of condition to make sure that you are suitable for the
treatment. The client shall provide, if needed, results of medical
examinations (X-ray, EKG etc.) or medical history performed at the
recommendation of Medica Travel.
The
Clinic or Hospital has the right to disqualify or stop a medical
treatment at any time without any obligation for compensation if
you are not mentally or medically fit for the suggested treatment
or if your payment obligations are not met. The Clinic or Hospital
can also postpone your treatment to a later date if new medical
information or condition occurs during the consultation or examination
that has not been known before by the doctor and can or might affect
the planned treatment negatively or put the Patient at a higher
risk. We cannot be held responsible if the Patient is refused the
treatment under such circumstances and you will not be compensated
for any financial losses. The Patient is able to complete additional
medical information, tests and book a treatment on a later date.
Offer
When the Clinic or Hospital has all necessary
medical information, pictures, tests and preliminary estimation,
Medica Travel will send the Patient an offer that states what Clinic
or Hospital the treatment is performed; name of dentist, doctor,
surgeon; qualification of the doctor; surgeon, scope of treatment;
price; payment information; recommendation of travel and accommodation,
when a treatment is possible etc.
The
Patient shall consult with Medica Travel regarding arrival and departure
dates and receive confirmation from Medica Travel before making
a reservation as every visit to Clinic or Hospital needs to be planned
in advance. Medica Travel does not sell air tickets or packages
including flights and hotels. However, we assist our customers in
finding flights and hotels.
Confirmation
If Patient is qualified for a medical treatment
and travel and accommodation are arranged, Medica Travel will send
the Patient confirmation of an appointment, treatment or surgery
by e-mail from Medica Travel. The confirmation states following:
scope of the treatment, schedule, prices and description of what
is included into the prices, payment method, how it works, how you
should prepare yourself before any planned treatment. The price
of treatment can change if something turns up during the consultation
with the doctor or dentist that has not been known before. The Patient
can always change the scope of treatment during the consultation.
The doctor or dentist will provide you with a treatment plan and
all costs explained before any treatment starts.
Cancellation
If cancellation rules are not specified in
the confirmation, you can at any time cancel your appointment, treatment
or surgery at no costs. The costs for tests, diagnoses, consultation
etc, if any, are paid by the Patient.
If
a confirmed appointment, treatment or surgery for any reasons from
the Clinic or Hospital side has to be cancelled; Medica Travel will
compensate the Patient's for actual costs as flight ticket and hotel
or a maximum amount of 500 zlotych. Patient should verify those
costs by sending copy of invoice, ticket or receipt showing costs
of flight and hotel.
Payments
All medical treatments are paid directly
to the Clinic or Hospital before, during or after the treatment.
All payment information about the Clinic and Hospital terms of condition
is specified in your offer and in the confirmation.
Payments
for travel and accommodation are paid separately to each provider
according to their terms of conditions.
Medical services
provided by Clinic or Hospital
All medical services are provided by specified
Clinics which take full responsibility for medical treatments performed.
Medica Travel is not responsible for any loss or damage occurred
to the Patient resulting from the performance of any of these services.
The contract for any medical service is directly between the Patient
and the Clinic or Hospital itself. Medica Travel is not liable for
the work of a doctor, dentist or any of the staff at the Clinic.
The contract between a Patient and Medica Travel is for the arrangement
of the services and other travel services specified on this webpage.
The
Clinic or Hospital will provide you with the aftercare instructions.
Patient's Responsibilities
The Patient is responsible for giving correct
and valid medical information to Clinic or Hospital. There is always
a risk connected with any treatment, even if the probability of
having a complication is low. A dentist, doctor or surgeon will
inform the Patient during the consultation what specific risks are
involved in the Patient's treatment. It is the Patient's own responsibility
to understand the risks involved in a treatment.
